Optum Peer Bridger

Based on our achievements with the OMH Peer Bridger Program, NYAPRS has partnered with Optum Health to provide peer Bridger services to individuals identified by Optum Health who are currently receiving mental health or substance abuse services in identified inpatient facilities or who are engaged in services at the identified out-patient facilities in NYC and Long Island.

The Optum Peer Bridger Program is designed to match a Peer Bridger who is successfully managing his or her own recovery, and has completed the requisite Peer Bridger training program offered by NYAPRS, with peers who have been identified by Optum Health and who are currently receiving services in identified inpatient facilities or who are engaged in services at the identified out-patient facilities.

The primary function of the program is the development of a supportive and trusting relationship between the consumer and the Peer Bridger. Peer mentoring, support, advocacy, and skill building will be provided for these peers through regular individual contact over a period of time with a goal of easing the transition of individuals being discharged from hospital settings back into community life and to significantly decrease the need for readmission to the hospital, and to significantly decrease the need for hospitalization by engaging people prior to entry into the inpatient facilities.

This service will promote the recovery model and provide tools for coping with and recovering from a mental illness and/or a substance use disorder.
