NYAPRS note: As all regions begin working within Health Homes, it's more important than ever that your agency and your service provision be prepared to succeed within this new environment. NYAPRS is continuing to offer you state of the art information regarding your possible role within Health Homes. Every 2-4 weeks we offer an informative webinar on topics of interest to the Collaborative by experts in the field. See below for details of our next meeting of our Health Home Learning Collaborative which has been designed to help agencies address improved engagement and service to communities of color.
