The Care Transitions Network offers targeted coaching, clinical consultation, and organizational trainings and resources to aid in implementing evidence-based practices and transition to value-based payments, short-term care transitions support to facilitate and ensure engagement with outpatient practices for people post discharge from psychiatric hospitalizations and a web-based platform for Quality Improvement to enable providers to track progress on key quality indicators.
